The American Society of Anesthesiologists (ASA) Task Force defined a difficult airway as “the clinical situation in which a conventionally trained anesthesiologist experiences difficulty with mask ventilation, difficulty with tracheal intubation, or both.” This article reviews the identification and preparation of these patients and discusses the various treatment options for managing the failed intubation. Modern airway management has evolved with the introduction of supraglottic devices as well as newer techniques to facilitate endotracheal intubation. Device options reviewed here include the fiberoptic bronchoscope, rigid fiberoptic laryngoscopes, the lighted stylette, GlideScope or Video Macintosh video laryngoscopic systems, and the ViewMax fiberoptic laryngoscope. Bag and mask ventilation devices include the laryngeal mask airway, the Combitube, and the Rusch Easy Tube. The difficult airway continues to be an unexpected challenge in acute care medicine. Having a plan for management can reduce potential complications, respiratory-related malpractice lawsuits and improve outcomes.

Video laryngoscopy in the prehospital setting.
In the prehospital setting, the emergency care provider must anticipate that some patients will manifest with difficult airways. The use of video laryngoscopy to secure an airway in the prehospital setting has not been explored widely, but has the potential to be a useful tool. This article briefly reviews some of the major video laryngoscopes on the market and their usefulness in the prehospit...

The appropriate airway management is the important point during the anesthesia. For it dose, ‎taking history, physical examination (head and neck, mandible, mouth opening, neck ‎movements …) must be done completely and they are helpful in estimating difficult ‎intubation.‎ It is necessary to note that one case from every ‎‏5000‏‎ cases of anesthesia, not o...

متن کاملUnanticipated difficult airway management in the prehospital emergency setting: prospective validation of an algorithm.
BACKGROUND Difficult intubation management algorithms have proven efficacy in operating rooms but have rarely been assessed in a prehospital emergency setting. We undertook a prospective evaluation of a simple prehospital difficult intubation algorithm. METHODS All of our prehospital emergency physicians and nurse anesthetists were asked to adhere to a simple algorithm in all cases of impossi...
متن کاملRapid Sequence Airway (RSA)--a novel approach to prehospital airway management.
This article presents a case in which an air medical flight crew encountered a potentially difficult airway when a trauma patient deteriorated in-flight. The crew elected to sedate and paralyze the patient and place a laryngeal mask airway without a prior attempt at direct laryngoscopy and endotracheal intubation. The term Rapid Sequence Airway (RSA) is coined for this novel approach. This arti...
